Individuals receive individualized proactive care and support in their own homes or at Ark Providence Homes, which is critical to reducing avoidable hospital admissions and improving care for vulnerable people. Our goal is to provide high-quality care for participants through collaboration among Ark Providence Registered Nurses, general practice, and frequent team meetings, designated clinicians, and shared projects. Our services are available 24 hours a day, seven days a week, including holidays. At any moment, our leadership team is accessible to discuss your concerns or answer any questions about your/participant's care. This service strives to improve independence and health outcomes by assisting people to remain at home through the NDIS.
